Permits Handicapped Parking Permit An application form is available through your physician, from your Town Clerk, the NYS Department of Motor Vehicles, or by selecting this link - Handicap Permit Application. The top portion of the form must be filled out in its entirety by the applicant and proof of identification will be required. A NYS drivers license or non-driver ID is acceptable. The medical portion must be filled out by the physician and his/her original signature must appear on the application. An applicant can obtain a temporary or permanent parking permit. This is determined by the physician. No fee is involved. One permit is issued per person. Application is made in the municipality where you reside.
