Parking Permit
An application form is available through your physician, from your Town Clerk, the NYS Department of Motor Vehicles, or by selecting this link - Handicap Permit Application.
- The top portion of the form must be filled out in its entirety by the applicant and proof of identification will be required.
- A NYS drivers license or non-driver ID is acceptable.
- The medical portion must be filled out by the physician and his/her original signature must appear on the application.
- An applicant can obtain a temporary or permanent parking permit. This is determined by the physician.
- No fee is involved.
- One permit is issued per person.
- Application is made in the municipality where you reside.
- Please note – if you reside within the Village of Hilton you can apply at the Parma Town Clerk’s office or the Village of Hilton Clerk’s offices.
